STANDARD SAFE DIVING PRACTICES STATEMENT OF UNDERSTANDING, VOLUNTARY RELEASE, WAIVER, EXPRESS ASSUMPTION OF RISK AND HOUSE RULES


This is a statement in which you are informed of the established safe diving practices for scuba diving.  These practices have been compiled for your review and acknowledgment and are intended to increase your comfort and safety in diving. Your signature on this statement is required as proof that you are aware of these safe diving practices.  Read and discuss the statement prior to signing it. If you are a minor, this form must also be signed by a parent or guardian.

am a certified diver, and I understand that as a diver I should:
  1. 1. Maintain good mental and physical fitness for diving. Avoid being under the influence of alcohol or dangerous drugs when diving. Keep proficient in diving skills and reviewing them in controlled conditions after a period of diving inactivity.

  2. 2.Use complete equipment with which I am familiar; and inspect it for correct fit and function prior to each dive. Recognise the desirability of an alternate air source and a low-pressure buoyancy control inflation system.

  3. 3. Listen carefully to dive briefings and directions from the skipper/Divemaster(s)/Instructor(s) and respect the advice of those supervising my diving activities.

  4. 4. Adhere to the buddy system throughout every dive. Plan dives - including communications, procedures for reuniting in case of separation, and emergency procedures - with my buddy.

  5. 5. Make all dives no-decompression dives and allow a margin of safety. Have a means to monitor depth and time under water. Limit maximum depth to my level of training and experience. Ascend at a rate of not more than 60 feet/18 metres per minute.

  6. 6. Maintain proper buoyancy. Maintain neutral buoyancy while under water.  Be buoyant for surface swimming and resting. Have weights clear for easy removal and establish buoyancy when in distress while diving.

  7. 7. Breathe properly for diving. Never breath hold or skip breathe when breathing compressed air and avoid excessive hyperventilation when breath-hold diving. Avoid overexertion while in and under water and dive within my limitations.

  8. 8. Not tease or harass, take with me any underwater organism dead or alive, but just observe.

I understand the importance and purposes of these established practices. I recognise they are for my own safety and well-being, and that failure to adhere to them can place me in jeopardy when diving.

Furthermore, I hereby affirm I thoroughly understand the hazards of scuba diving including those hazards occurring during boat travel to and from the dive site.  I understand that these hazards include, but are not limited to, air expansion injuries, drowning, decompression sickness, slipping or falling while on board, being cut or struck by a boat while in the water, injuries occurring while getting on or off a boat, and other perils of the sea. I understand and agree that neither Big Blue Divers, nor any of their respective owners, employees, instructor(s), divemaster(s), agents, contractors or assigns may be held liable or responsible in any way for any injury, death or other damages to me, my family, estate, heirs or assigns that may occur as a result of my participation in diving and diving related activities.
 

KINDLY NOTE THE FOLLOWING:

  • Whether I have purchased my dives in advance, or have an open account, I understand that I will need to pick the dive excursions I would like to join, and make a reservation accordingly at the reception.

  • I understand that travelling by boat can cause seasickness, and that I should take any preventive aids before my boat departure should there be a possibility that I can become seasick during the boat trip. I understand that if I choose not to use prevent from getting seasick, or for any other reason I am unable to participate in the dive, and need to stay on board, I am paying for the boat trip and rental of equipment.

  • I hereby assume financial responsibility for any of my own equipment, or rental equipment that I may lose or may become damaged.

  • Kindly hand over rental dive computers, compasses, masks, cameras to your Divemaster/Instructor or at the reception only! Failure to do so might result in charges in case any item is lost! Thank you for your cooperation and understanding.

  • Herewith I am advised that general personal belongings can be taken on board, but it is my responsibility to ensure they stay dry and are properly protected from water, and that I take them with me from the boat after my diving activities for the day. Any valuables not to be used during your excursion should be kept safe in one of the lockers at the reception in the dive centre.

  • Herewith I am informed that diving cancellations due to any reason need to be made one day before scheduled reservation, cancellations made 12 to 15 hours before scheduled departure are subject to a 50% cancellation fee. No shows or cancellations made within 12 hours of scheduled departure are charged 100%.

  • I am aware, and I agree to pay any additional costs that apply to the diving excursions in which I participate, such as; entrance to the marine park, and fees for night, long and extra-long distance boat trips, day trips, EANx, etc.

  I have read the above statements and have had any questions answered to my satisfaction.
